Introduction

Myriad provides an assortment of tools for patient identification and education. These tools can be branded with your contact information. Outreach materials are available for print on a password-protected section of Myriad's website. Myriad requests that any re-prints that contain Myriad logos, names, trademarks or other information identifying Myriad not be materially changed unless the Myriad logos, names, trademarks or other information identifying Myriad is removed. Please print and sign this Letter of Agreement (pdf) agreeing to the above statement. Please fax the Letter of Agreement to (801) 883-3256. Once Myriad receives the fax we will contact you with the user name and password.

The files are created in Quark Express 4.0. Any professional printer will be able to access the files. You should provide the printer with your logo in .eps or .tiff format. He or she will then be able to import your logo into the designated area on each collateral piece. Be sure to have the printer delete the rectangle with the "your logo here" written on it. This is an important step, otherwise the rectangle will print along with your logo. Once these steps have been completed, the printer will be ready to print the documents.
