Appropriate patients should be educated and counseled
regarding genetic testing for hereditary cancer. You may want to
use Myriad's
Patient Education Tools, a series of patient's guides to risk
assessment and genetic testing. These educational pieces will guide
your discussion with patients on the hereditary cancer syndrome
and covers the key information necessary to obtain informed consent
for genetic testing. It may be provided to patients as a
summary of their visit. More details on the education process are
covered in detail in the disease-specific section of this Resource
Guide. After education and counseling, the patient needs to read,
understand and sign an Informed
Consent Form (pdf).
The form describes limitations and benefits of genetic susceptibility
testing. It also reviews Myriad's test cancellation and financial
responsibility procedures. The Informed Consent Form is provided
as part of this Resource Guide and is in every test kit. Myriad
does not require the actual consent form, but rather a signature
on the Test Request Form that confirms that informed consent was
obtained. The signed Informed Consent Form can be placed in the
patient's medical chart.
Additionally,
the Hereditary
Cancer Education and Testing Process Checklist (pdf)
is a simple form that can be placed in the patient's chart,
documenting what activities have been completed with the patient,
where they are in the testing process, and what still needs
to be done. This is an easy and convenient way to monitor each
patient through the process.
