patient in your practice should be screened for hereditary cancer
risk. Those patients identified to be at increased risk may go on
to have further risk assessment. Ideally, patients should be screened
on an annual basis, since their personal or family history of cancer
may change. Many tools are available to simplify the process of
patient identification. Some tools are listed below, and you may
wish to speak to your Myriad representative to determine which tools
are best for your office.
Tools for the Healthcare Provider include the:
- Red
Flags Flashcard (pdf) –
a quick reference card with information on which patients
may be at risk for hereditary cancer syndromes. These Flashcards
are appropriate to share with your office staff and referring
physicians in your area.
- Post-It
Note (pdf) – allows
your office staff to flag charts of patients who may be at
risk for a hereditary cancer syndrome.
- Referral
Pad (pdf) – allows
you to easily refer your patient to a genetic counselor at
your local high risk assessment program if you choose not
to offer this service in your own office.
- Fax
Notification Sheet (pdf) – allows
you to send a patient's information to your local high risk
assessment program if you choose not to offer this service
in your own office.
Tools that encourage patient self-identification and
education include posters and brochures for your waiting or exam
rooms. These posters and
brochures are appropriate
to share with referring physicians in your area.
Tools that aid in collection of
family history information include family history questionnaires.
These brief family
history questionnaires can be provided to every patient to
help you identify which patients need additional risk assessment
for a hereditary cancer syndrome. These questionnaires may also
be provided to referring physicians in your area.
